Adoption Agreement

This Agreement is entered into this date by and between Dauntless Dog Rescue of NC (DDRNC) and the applicant
(“Adopter”) for the provision of adoption of a rescued dog or puppy, hereinafter referred to as “dog”.

Terms and Conditions:

1. Adopter agrees to pay an adoption fee of $_____ for ________________. This amount is considered a donation and is
nonrefundable unless agreed-upon in very specific circumstances by the board of DDRNC. All returned checks are subject
to a $35 fee. DDRNC has the right to take the dog back immediately upon failed payment.
2. Adopter agrees to have open communication with DDRNC, to return phone calls and to honestly answer our questions when
we call to check on the animal’s well-being. Additionally, DDRNC reserves the right to conduct a home visit within thirty
(30) days of the date of this Agreement.
3. Adopter agrees that if for any reason you can no longer provide a home for this dog you will contact DDRNC and will not
for any reason at any time take the dog to an Animal Shelter or Animal Control. Adopter realizes that they will need to keep
the dog in their possession until a new foster or adoptive home has been found. Adopter agrees not to give away, sell or
dispose of the dog without express consent of DDRNC. DDRNC reserves the right to check up to ensure that you are still in
possession of the dog.
4. Adopter agrees the dog is to be a family companion and to provide the dog with a safe home, adequate food, water, proper
care, exercise, love and attention. Adopter agrees not to violate any laws or ordinances with the dog. The dog is not to be
used as an attack or protection trained dog and will not be used for commercial security work.
5. Adopter agrees to complete vaccinations and other required medical care at their own expense from this point on.
Additionally,, adopter agrees to have the dog examined annually by a licensed veterinarian with first vet visit being within
30 days of adoption, and to provide vaccinations, monthly heartworm preventative, rabies shots (according to the law) and
any additional medical care deemed necessary.
6. DDRNC makes no claims or guarantees in regard to this animal’s age, breed, health or temperament and is NOT liable for
any future injury or damage that may be caused by this animal. Adopter accepts this animal as is with all defects, either
observable or unobservable, and assumes all risk for the dog upon signing of this Adoption Contract.
7. Adopter certifies that information provided on the Adoption Application is true and complete. You understand that
falsifying information on the Adoption Application or at any other time during this process disqualifies you from fostering.
8. Adopter certifies that no one residing in the household with the dog has ever been convicted of Animal Cruelty, Neglect or
Abandonment.
9. Adopter agrees to all terms in this agreement or DDRNC will take back possession of the dog without refunding the
adoption fee or other expenses paid by the adopter.

Release of Liability:

I have read and fully understand the Adoption Agreement. I understand that the adoption of this dog from DDRNC is at my
own risk and agree to indemnify and hold DDRNC, DDRNC’s volunteers and any other DDRNC representative(s) harmless
for any illness that the dog may develop and/or any injury or damage this dog may cause once I have taken possession of the
dog.
